Last Friday sent Urban Decay's customer service an email inquiring about their makeup artist program and here was the response that I received from them.
"Thank you for your interest in representing our brand. We do have an informal make-up artist discount program set up. If you would like to be considered for a discount on our website, please fax your resume and credentials to 949-515-5614 Attn: Jesse. We will review your information and get back to you shortly. The discount offered is 20% for all qualifying and approved make-up artists. Have a wonderful day."
"Thank you for submitting your resume for our Make-up Artist Discount Program. Your request has been approved. At the present time, we do not have a formal discount program setup. However, we are currently working on this and we’ll send you more details once it’s up and running. As of now, please enter the code [CODE WAS HERE] into the coupon box during checkout to receive 20% off your purchase. Please note that this code is to be used only once. If you wish to place another order with us, please contact me directly. This code is for your personal use only and should not be shared with anyone. If we find that you are in violation of these terms and agreements, your discount privileges can and will be revoked. If you have any further questions, please feel free to write back or give me a call toll-free. Thank you and have a great day!"
QUICK TIP: ALWAYS have a cover letter if you are faxing something. It should state your contact information, the company you're sending it too, who it should be sent to the attention of and how many pages you are sending. So in case something happens the person who is receiving it knows how many pages you intended to send so if one page is missing they can contact you asking to re-send the info. Here is a sample of mine that I sent to UD, obviously information has been edited for the sake of my own privacy.

Pretty easy stuff. I am also sending out applications to a few other brands, I'll let you know the process with each particular brand once I know if I have been approved or denied (I BETTER NOT BE! haha)

So, with the Mac Pro membership, they ask for a few identifying pieces of information. What did you use? I do freelance, but you don't have to attend a school to do it in my state. Any information would be helpful. Thanks (edkdolce@gmail.com)

I went to cosmetology school, but I never use my documents/certificates from that if I apply for makeup discounts. Because a lot of companies give hair stylists and other professionals discounts but at a lesser rate than makeup artists. And typically cosmetologists only do hair, at least here so I didn't want to risk the chance of getting a lesser discount because of my cosmetology training in assumption that I am a hair stylist when I am a practicing makeup artist.
For MACPro I sent in a contract from a wedding I had done and a business card. They don't specify if your contract needs to be a completed contract or blank. I had a client who was fine letting me use her contract as long as I blocked out her address and contact information.
